First AWAH Z2 Improper Operation Accident Report
On April 10, 2023, an air conditioner installer dropped an outdoor air conditioner unit while lowering it into place using the AWAH Z2 for lifting. Fortunately, no personnel injuries or casualties were sustained in the incident.

Incident Description: The operator directly pulled the handle to unlock, causing the load to fall to the ground. Although the operator was wearing gloves, they could not maintain a firm grip on the control end of the rope, and the gloves were worn through.

Cause Analysis: The operator did not read the instruction manual before use. When lowering the heavy load, the rope was not wrapped around the friction hooks as required. Under this condition, the Z2 functions as an efficient pulley rather than a descender, so there is insufficient friction to allow single-hand control. (Note: Most descenders without anti-panic features carry a risk of loss of control if the handle is pulled fully open when lowering heavy loads!)

Serious Warning: Be sure to read and fully understand the entire AWAH Z2 instruction manual, master all safety operation requirements, and receive targeted training. Do not perform work at height until you are proficient in its operation. Work at height concerns life and property safety and must never be taken lightly! The Z2 must be used with a backup safety system for safe operation.
